Upon making your appointment, we will text you a confirmation and email simple forms to fill out online. This is the preferred method to promote efficiency and minimize your wait time.
If you prefer, we have downloadable patient forms that you can print and fill out before your next visit with Boerne Vision Center.
If you choose not to fill out paperwork beforehand, please arrive 15 min early to complete paperwork.
